On our script do we need to include small actions that's being done like a real play script or do just want the conversation like the example you give us? For example do we need to mention that the child is playing witha toy or just strictly stick to what's being said?

You only have to transcribe what the child says. If you feel like some action is important to what is being said you can certainly include it.
